Assisted living or a nursing home?

The dividing line in Tennessee is continuous nursing care, not age or diagnosis.

Quick answer

When they need continuous nursing care, meaning round-the-clock observation, assessment or nursing services only a licensed nurse can perform, or have another condition Tennessee's ACLF rule bars, such as stage III or IV pressure ulcers or a need for restraints (0720-26-.08(1)).

When does someone need a nursing home instead of assisted living in Tennessee?

When they need continuous nursing care, meaning round-the-clock observation, assessment or nursing services only a licensed nurse can perform, or have another condition Tennessee's ACLF rule bars, such as stage III or IV pressure ulcers or a need for restraints (0720-26-.08(1)).

The line is not as fixed as many families assume. Tennessee lets an ACLF admit or keep someone who meets nursing facility level of care if the treating physician certifies in writing that the person's needs can be met there and the facility can assure timely evacuation. A parent who qualifies for a nursing home on paper may still live well in assisted living with outside home health or hospice support.

How do costs compare in Tennessee?

CareScout's 2025 Tennessee medians are about $5,595 a month for assisted living and $9,429 a month for a semi-private nursing home room ($10,038 private), so a nursing home costs roughly 70% more at the median.

Payment differs too. TennCare CHOICES Group 1 pays for long-term nursing home care for eligible residents, who keep $50 a month. In an ACLF, CHOICES Groups 2 and 3 can pay for ACLF services but not room and board. Medicare pays neither long term, but covers up to 100 days of skilled nursing after a qualifying hospital stay.

How is quality information different?

Certified nursing homes have CMS Care Compare star ratings, inspection reports, staffing hours and turnover data; Tennessee assisted living has no official rating, so families rely on HFC's license listing, last survey date, disciplinary actions and the facility's own survey reports.

Both sets of records are on our facility pages. For nursing homes, read staffing hours per resident day and turnover alongside the stars; for ACLFs, ask for the latest statement of deficiencies and plan of correction.

Can a Memphis assisted living resident move to a nursing home and come back?

Sometimes. A short nursing home or rehab stay after a hospital admission does not end an ACLF residency by itself, but the resident can return only if the ACLF can again meet their needs safely under Tennessee's admission rules, so ask the ACLF whether it will hold the apartment and for how long.

Holding an apartment usually means paying rent during the absence. Ask in advance how the facility handles a resident who comes back needing more help, and whether it will reassess the care level and price.
